Job Description

The LCSW-Social Worker for Case Management provides psychosocial support to patients with complex needs, collaborating with a multidisciplinary team to ensure safe discharge planning and continuum of care.

Offering a $3,000 sign-on bonus, the role requires a Master's Degree in Social Work, an LCSW license, and current BLS certification. Preferred candidates have at least one year of supervised clinical experience in a medical setting.

Responsibilities include comprehensive assessments, crisis intervention, and referrals, utilizing knowledge of psychodynamic and crisis theories, brief counseling, and community resources.

This full-time position offers hourly pay between $40.00 - $48.10 and benefits such as health, dental, vision, life insurance, flexible spending, and paid time off.

Work is conducted in person to support patient care and discharge planning.
